Most treatments can begin the same day, though a complex medical history or treatment plan may require an evaluation and a follow-up appointment on another day.
If your dentist has existing x-rays, please have them forwarded or bring them with you. Additional x-rays can be taken at our facility if needed.
Please provide a full list of your current medications. Alert us to any conditions such as diabetes, high blood pressure, artificial heart valves and joints, or rheumatic fever.
Any x-rays taken by a previous dentist (physical or digital), if applicable.
Bring your necessary completed forms so we can help you process any claims.
Patients under the age of 18 must be accompanied by a parent or legal guardian.
